Dear The Quaker,
I'm a physician working on the Labor and Delivery floor of a large university hospital which shall remain unnamed. A recent delivery has raised a burning question in my mind.
There I was, in a delivery room with a very nice young lady. I had my two fingers in her vagina, pressing on her perineum to help her feel through the epidural anesthesia where she should push.
Encouragingly, I said "There you go "patient X" that's good. I want to you really try to push on this next set of contractions and we'll see if we can get this baby delivered."
Her question both stunned and stymied me. She simply asked me, "Push in or out?"
I was too baffled to respond. My question to you, Quaker, is what happens when you push in?
